I was in my first year of practice- and had never really dealt with the issue in school- when I had encounter with a German lady who insisted on being massaged without a sheet because she was from germany and was used to not being covered. I baulked at first, but she upbraded me, telling me that she knew I was a professional and had to tell myself that when I was in uniform I was dedicated to my craft above all distractions! I agreed, realising that I was allowing not so much a genuine apprehension about my ability not to be distracted by female nudity (or erections, or whatever), but rather a conditioned rigidity and fear about lack of control and unfounded issues about decorum and manipulation. For the first few months I was so 'concerned' about 'it' that I would even leave the room when the client was turning over! This persisted to the patient consternation of my first clinic director until I had a woman in the Spa I was working get up and throw her sheet off after I was done doing a short massage. She upbraded me for allowing stupid hang-ups to get in the way of providing a service from one professional to another, and reminded me that if she the client had full trust in me then I'd better reciporicate that trust with more self confidence!
